Book A VitalityHealth Face to Face GP Appointment

Privé Health is proud to provide a face to face GP service to members of VitalityHealth. Before booking an appointment, please ensure that your membership entitles you to this service, and take care to note the following information:

1. Vitality GP Benefit Limit

VitalityHealth provide a Vitality GP benefit limit of £100.  This covers at least two consultations at a reduced rate of £45.00 as well as any primary care tests and prescriptions that the GP prescribes. You will have to make a £20 payment towards the cost of the consultation and the remaining £25 will be deducted from your benefit limit.

2. Additional Services

You will also be asked to pay the GP surgery directly for any additional primary care treatment they recommend and you can then claim this back from VitalityHealth provided that you have sufficient Vitality GP benefit limit remaining.  You are responsible for ensuring that you have sufficient benefit limit to cover the cost of any consultations or treatments.

1. Confidentiality
  1. Please refer to our Privacy Policy on our website.

2. Fees
  1. Any fees incurred will be your personal liability; we recommend you ensure that you are familiar with our scale of fees and discuss the likely cost of investigations with the treating clinician before they are undertaken. You will also be asked to pay the GP surgery directly for any primary care treatment they undertake.

3. Insurer Cover
  1. Vitality Health provide a Vitality GP benefit limit of £100. This covers at least two consultations at a reduced rate of £45.00 as well as any primary care tests and prescriptions that the GP prescribes. You will have to make a £20 payment towards the cost of the consultation and the remaining £25 will be deducted from your benefit limit.  You will also be asked to pay the GP surgery directly for any primary care treatment they recommend and you can claim this back from Vitality Health provided that you have sufficient Vitality GP benefit limit remaining.  You are responsible for ensuring that you have sufficient benefit limit to cover the cost of any consultations or treatments.  If you are not sure you should contact Vitality Health on Individual/Personal Healthcare plans: 0345 602 3523 Business Healthcare plans: 0345 602 3437 Corporate Healthcare plans: 0345 279 8865.

  2. We will require details of a credit card / debit card to cover any fees arising in the event of a late cancellation or missed appointment charge, or should any other outstanding debts become overdue. In order to activate your card, we will take a £20.00 fee at time of booking your appointment. Should you go on to cancel your appointment (within the correct timeframes) this will be refunded to you subject to a £5.00 administration fee.

  3. We reserve the right to refuse a booking in cases where a valid credit or debit card can not be obtained.

4. Cancellations and rescheduling
  1. Please note that we reserve the right to charge fees in accordance with our cancellation policy for non-attendance and late cancellations or failure to attend appointments without notification.

  2. If you arrive after your appointment start time you may be asked to reschedule your appointment if the clinician is fully booked and cannot fit you in. Please be aware that if you are using insurer cover, your insurer will not cover late cancellations / rescheduling or DNA (Did Not Arrive) charges.

  3. The minimum notice period for cancellations and rescheduling is 24-hour before the start time of your appointment.

  4. Notice less than 24 hours or a DNA will be charged as a full consultation.

5. Payment terms
  1. Prive Health and our partner clinics policy is to require full payment at time of appointment on all services provided above the consultation fee. A valid credit or debit card will be authorised at time of booking for £20.00 which will constitute as your consultation fee contribution. Subject to available member benefit remaining, Vitality Health will pay the remaining £25.00. If you do not have available member benefit your credit or debit card will be billed for the remaining fee.

  2. Vitality Health have pre approved a number of tests for which they will (subject to available remaining member benefit) reimburse you. It is not the responsibility of Privé Health or its partner clinics to reimburse you. Privé Health and its partner clinics hold no responsibility to you, the patient, for the accuracy of which tests may be reimbursed, as these may change from time to time.   3. To claim reimbursement, you are required to email your Privé Health receipt to vitalityGP@vitality.co.uk along with your member number.

  4. Once you have used up your remaining member benefit within a calendar year, Privé Health and our partner clinics reserve the right to charge our standard tariff of fees. Details of which can be found on our website www.privehealth.com

  5. Interest will be charged at the rate of 4% per annum above Barclays Bank’s base rate from time to time on all non payments, calculated from the date of your visit.

  6. Any further collection costs incurred by Prive Health including but not limited to, collection agencies, credit reference agencies and solicitors will be added to the final amount payable.

6. NHS GP
  1. Privé Health is unable to offer services which are available through the NHS (e.g. GP Consultations) at the clinic where you are registered as an NHS patient. It is your responsibility to ensure you comply with this requirement.
